Registration

Step 1:
Download the registration packet, which includes the Employer Tools Enrollment Form and Billing Agency Authorization Form.

Step 2:
Complete the requested information.

Step 3:
Mail the registration information to:

You can also fax the form directly to the EDI Coordinators at 1-508-368-9996.

Upon receipt, Fallon Health will verify your status as an employer and assign you a username and password.
