Fallon has partnered with hospitals, health care providers and community health centers across the state to offer three MassHealth ACO plans:
Berkshire Fallon Health Collaborative
Available in parts of western Massachusetts, Berkshire Fallon Health Collaborative is a partnership with Berkshire Health Systems, Community Health Programs and several Berkshire County community physician practices.

Available in parts of central and eastern Massachusetts, Fallon 365 Care is a partnership with Reliant Medical Group.

Fallon Health-Atrius Health Care Collaborative
Available in parts of central and eastern Massachusetts, Fallon Health-Atrius Health Care Collaborative is a MassHealth Accountable Care Organization (ACO) Partnership Plan. It’s made up of Atrius Health, Fallon Health and other select community providers. All are health care leaders who have been caring for MassHealth members for decades.
