Karen Coderre, Pharm.D.

Karen Coderre, former Senior Director of Pharmacy at Fallon Health, is an experienced managed care and board-certified psychiatric pharmacist.

Prior to working at Fallon, Coderre worked at Health New England, where she developed and managed medication therapy management programs for Medicare, Medicaid and commercial business lines.

At the University of Massachusetts Medical School, she integrated pharmacy services and medication therapy management into an established case management program for medically complex individuals. Earlier in her career, she was Director of Pharmacy for McKesson Medication Management.

Coderre holds a Doctor of Pharmacy from the University of Florida, along with a master’s in hospital pharmacy from Northeastern University. She holds undergraduate degrees in pharmacy and biology from the Massachusetts College of Pharmacy and Health Sciences and the University of Massachusetts at Amherst, respectively.

Sheila Despres, PT

Sheila Despres, a licensed physical therapist since 1984, specializes in working with older adults. She is currently the Site Director for the Leominster location of Fallon Health’s Summit ElderCare program, which is a Program of All-Inclusive Care for the Elderly (PACE). 

Previously, Ms. Despres served for 10 years as Director of Rehabilitation Services for all Summit ElderCare locations, overseeing a staff of 32 physical and occupational therapists and managing speech therapy services.

Lisa DeWitt, RN, LNC

Lisa DeWitt is the Senior Director of Operations for Fallon's Summit ElderCare program. In addition to having extensive experience as a home care nurse manager, she previously served as Clinical Nurse Manager for Fallon Health’s NaviCare program and as Site Director of Summit ElderCare in Charlton, Massachusetts.

A registered nurse, Ms. DeWitt holds a bachelor’s degree in mental health and psychology, with a specialty in behavioral health and certification in wound care. As a legal nurse consultant, she reviews medical cases and provides expert witness testimony during relevant court cases.

Heather Dobbert, M.S.W, LICSW, CDP

Heather Dobbert is a Behavioral Health Case Manager and Memory Specialist for Fallon Health. She works with Fallon members living with various behavioral health issues, including dementia. She also provides resources and support for their families. Her goal is to develop personalized interventions to improve the caregiving experience while enhancing quality of life for the person living with dementia. In this role, Ms. Dobbert works in partnership with experts at the Alzheimer’s Association to ensure that members and their families have direct access to best practices and resources in dementia care.

Ms. Dobbert, a licensed independent clinical social worker, serves as a trainer for the National Council of Certified Dementia Practitioners and also teaches the Council’s Alzheimer’s and Dementia Care Seminar. She is also a support group leader and volunteer for the Connecticut and Massachusetts chapters of the Alzheimer’s Association.

Vickey Ferrante, RD

Victoria Ferrante served as a Disease Management Health Educator at Fallon Health until 2020. In addition to being a registered dietitian, she is a certified diabetes educator.

At Fallon, Ms. Ferrante coached, educated and motivated members with diabetes to achieve their individual goals for managing their conditions. She also connected members with other Fallon resources available to help them. She is passionate about advocating for and educating people with diabetes. As someone with type 1 diabetes herself, Ms. Ferrante knew from an early age that she wanted to empower others to live well with diabetes.

Her additional health care experience includes positions as a Clinical Dietitian in hospital, skilled nursing and long-term acute care/rehab settings. She has also provided outpatient diabetes and nutrition education. Ms. Ferrante holds a Bachelor of Science degree in food and nutrition from Framingham State University, where she is now a guest lecturer for the Coordinated Program in Dietetics.

Karen Gagliastre, M.Ed., CHES

Karen Gagliastre joined Fallon Health in 2011 and served as Manager of Health Promotions until 2020. As a Certified Health Education Specialist (CHES), she provided wellness education opportunities to improve health outcomes for Fallon members and others in the communities we serve.

Prior to coming to Fallon, she was a dental hygienist for 20 years and then served as Health & Wellness Director at the YMCA of Central Massachusetts, Central Community Branch for several years.

Ms. Gagliastre holds an associate’s degree in dental hygiene from Quinsigamond Community College, a bachelor’s degree in community health education from UMass Amherst and a master’s degree in health education from Worcester State University.

Gerald Gleich, M.D.

Dr. Gerald Gleich is a family physician specializing in geriatrics. An associate clinical professor of Family Medicine and Community Health at UMass Medical School, Dr. Gleich has dedicated much of his career to caring for older adults and has been a clinician-educator in family medicine and geriatrics since 1987. He served as the Medical Director for Fallon Health’s NaviCare program, a Senior Care Options/Medicare Advantage Special Needs Plan, and retired at the end of 2019.

Dr. Gleich is also the Medical Director at St. Mary Health Care Center in Worcester, where he maintains a practice as a long-term and post-acute care clinician and educator.

Mary Ann Graham, M.S., RN, LDN

Mary Ann Graham is Site Director at Fallon’s Summit ElderCare location in Webster. In addition to overseeing daily operations of the facility, Ms. Graham leads an interdisciplinary team of professionals who specialize in geriatric care. This ensures that each participant has a personalized care plan to help them live in the community as independently as possible, rather than in a nursing home.

Prior to joining Fallon as Site Director, Ms. Graham was the Manager of Quality and Compliance at the Elder Service Plan of Cambridge Health Alliance. She also worked as a registered dietitian at Reliant Medical Group.

Ms. Graham holds two master’s degrees—one in nutrition from Framingham State University and another in management of aging services from the University of Massachusetts, Boston.

Mojgan Sanepour Haji, Pharm.D., RPh

Since 2016, Mojgan Haji has been a Clinical Pharmacist in Fallon’s Safe Transitions program, which helps recently hospitalized patients with their home medication regimens. She has extensive experience with medication therapy management, pharmacy operations and care transitions. Ms. Haji worked at Charlton Memorial and St. Vincent hospitals from 2014 to 2016, and before that held several pharmacy-related roles at Fallon.

Ms. Haji holds a Doctor of Pharmacy from Idaho State University and Bachelor of Science in Pharmacy from Howard University in Washington, D.C. She is member of the American Society of Consultant Pharmacists and the Massachusetts Pharmacists Association.

Brenda Juskavitch, CTTS-M

Brenda Juskavitch, Health Promotions Supervisor, has worked for Fallon Health since 1995. With wide-ranging expertise in health coaching and tobacco cessation programming, she provides comprehensive health coaching services, counseling and education to Fallon members. Tapping into her extensive motivational interviewing skills, Brenda helps members identify their health goals and make healthy lifestyle choices. She obtained certification as a master-level tobacco treatment specialist from the University of Massachusetts in 2008.

As a founding member of Fallon’s Council for Cultural Competence, Brenda strives to raise awareness of the value that individual differences can have in the workforce. She believes that exploring these differences through education and discussion is essential to creating and maintaining a respectful environment for all employees. She continues to support the Council’s mission as a steering member.

Brenda J. King, Psy.D.

Dr. Brenda J. King is a licensed clinical psychologist specializing in health psychology, midlife and geriatrics.

She studies and treats the emotional and psychological effects of aging, medical illnesses and caregiving.

Since 2018, Dr. King has been Director of Behavioral Health Services for Fallon Health’s Summit ElderCare program, a Program of All-Inclusive Care for the Elderly (PACE). She served as a Behavioral Health Specialist for Summit ElderCare for five years before taking on the role of Director. She also sees patients in her private practice at Grove Counseling Services in Worcester, Massachusetts, and teaches in the Graduate Studies in Counseling Psychology Program at Assumption College.

She holds doctoral and master’s degrees in clinical psychology from Antioch University New England, Keene, New Hampshire, and a bachelor’s degree and certificate in gerontology studies from Assumption College, Worcester. She completed her post-doctoral training at UMass Memorial Medical Center, also in Worcester.

Jill Green Lebow

As Fallon’s Senior Vice President and Chief Human Resources Officer, Jill Lebow ensures that Fallon attracts, engages and develops the talent necessary to meet its growing and changing organizational and business needs.

Ms. Lebow joined Fallon as a Human Resources Business Partner in 2008. She held a similar role at Hanover Insurance and also worked in employee relations at AbbVie pharmaceuticals.

She has a bachelor’s degree in English literature from the University of Connecticut and a Juris Doctorate from University of Connecticut School of Law.

Emily Lemire, M.S.W., LICSW

Emily Lemire has been Social Work Manager for Fallon Health’s Summit ElderCare location in Lowell since 2016.

A licensed social worker, Ms. Lemire began working with older adults in 2000 and has 10 years of home health and hospice experience. She also has an extensive background in long-term care and protective services.

She holds a master’s degree in clinical social work from Boston University.

Katherine Loomer, Pharm.D., BCACP

Katie Loomer has been a Clinical Pharmacist in Fallon Health’s Safe Transitions Program since 2017. In her role, she helps recently hospitalized patients with their home medication regimens.

Ms. Loomer had experience with home visits and care transitions in an earlier position as a clinical pharmacist with Dovetail Health. Prior to joining Fallon, she worked as a clinical pharmacist for six years in Atrius Health’s primary care physician offices.

After earning her Doctor of Pharmacy degree from the University of Rhode Island, Ms. Loomer completed two years of post-graduate residency training at the University of Pittsburgh Medical Center St. Margaret Hospital. She holds board certification as an ambulatory care specialist through the American College of Clinical Pharmacy.

Michelle Malkoski, RN

Michelle Malkoski has been the Director of Quality and Risk for Fallon Health’s Summit ElderCare program since 2017.

A registered nurse, Ms. Malkoski began working in senior health care in 1994. She has an extensive background in long-term care nursing administration.

She holds a bachelor’s degree from Eastern Nazarene College, in Quincy, Massachusetts.

Leanna Moran, M.S.W.

Leanna Moran is Site Director of Fallon Health’s Summit ElderCare location in Lowell on the campus of D’Youville Life & Wellness Community. Since joining Fallon in 2011, Moran has held various leadership roles in senior care services. As Executive Director of Fallon’s NaviCare program, she led the program’s overall operations, growth and continued expansion. She also served as Director of Outreach for NaviCare and Summit ElderCare.

In her current role, Ms. Moran oversees the daily operations of our Lowell Summit ElderCare program, a Program of All-Inclusive Care for the Elderly (PACE). She leads an interdisciplinary team of professionals who specialize in geriatric care and ensure that participants have personalized care plans designed to help them live as independently as possible.

Previously, Ms. Moran worked as corporate marketing director for Senior Living Residences, an assisted living provider specializing in Alzheimer’s care. She also owned and operated her own guardianship and care management business, serving elders throughout Massachusetts.

Ms. Moran has a master’s degree in clinical social work from Boston College and a bachelor’s degree in psychology from Brandeis University.

Diane Paseos, M.S.W., LICSW

Diane Paseos, a licensed clinical social worker, is a Behavioral Health Case Manager for Fallon Health’s Clinical Integration program. Prior to joining Fallon, Ms. Paseos was a Clinical Supervisor and coordinated specialized geriatric outpatient services at an outpatient mental health agency. Ms. Paseos has also provided therapeutic and consultative services to skilled nursing and long-term care residents and facilities. Ms. Paseos takes a trauma-informed and patient-centered approach to helping members.

Ms. Paseos holds a master’s degree in clinical social work from Fordham University, a bachelor’s degree in social work from St. Joseph College, West Hartford, and a certificate in geriatric care management from Boston University. She is a Certified Dementia Practitioner and holds an advanced training certificate in Trauma-Focused CBT. Ms. Paseos is an M.B.A. candidate at UMass Amherst’s Isenberg School of Management, with a concentration in Healthcare Administration.

Lynn Patterson, RN, CCM

Lynn Patterson has experience caring for patients as well as directing clinical and business operations in the health care industry. She currently serves as Senior Director of Clinical Services for Fallon Health’s NaviCare program, a Senior Care Options/Medicare Advantage Special Needs Plan. In that role, she oversees clinical service provision, quality management, member services, utilization management, case management, network management and benefit coordination.

Since 1998, when she joined Fallon, Ms. Patterson has held multiple case management and utilization management positions. Prior to coming to Fallon, she worked in the skilled home health care industry.

Certified in case management since 2000, she holds a bachelor’s degree in nursing from Salem State College. 

Laura Roias, LICSW

Laura Roias, a licensed social worker, is the Behavioral Health Manager for Fallon Health’s NaviCare program. Ms. Roias has worked in behavioral health since graduating from the University of North Carolina in 2006 with a master’s degree in social work. She has experience with adults and families in both inpatient and outpatient settings. Prior to joining Fallon Health in 2018, Ms. Roias directed two outpatient mental health clinics in Central Massachusetts. She employs a strengths-based approach to helping members.

Annamaria Stamos, M.P.A.

Annamaria Stamos has extensive experience in health care, including operations, financial management, utilization and quality management. She is currently Executive Director of Fallon Health’s Summit ElderCare program, a Program of All-Inclusive Care for the Elderly (PACE). Prior to joining Fallon Health in 2010, she spent 18 years working in the behavioral health care field.   

Ms. Stamos earned her master’s degree in public administration at the University of Rhode Island and a bachelor’s degree in clinical/medical social work at Rhode Island College. As an emergency medical technician (EMT), she served as a volunteer rescue lieutenant for 16 years for the West Glocester Fire Department, also in Rhode Island.
