Robert Schreiber, M.D. named Vice President and Medical Director of Fallon Health's all-inclusive care program for older adults

Worcester, Mass., Jan. 2, 2018Fallon Health, a nationally recognized, not-for-profit health care services organization, is pleased to announce Robert Schreiber, M.D., as Vice President and Medical Director of its Program of All-inclusive Care for the Elderly (PACE).

Dr. Schreiber brings more than 30 years’ experience and expertise in geriatric medicine and coordinated care to Fallon with specialties in prevention and wellness strategies for older frail adults, palliative care, transitions of care as well as connecting community-based organizations with health care providers.

At Fallon, Dr. Schreiber is responsible for supervising the medical care delivery of the 1,220 participants enrolled in the organization’s six PACE locations in Massachusetts—known as Summit ElderCare™ — and its Fallon Health Weinberg-PACE in New York. The primary goal of PACE is to ensure that adults age 55 and older age in place—or live as independently and safely as possible in their own homes.  

Prior to joining Fallon, Dr. Schreiber was Medical Director of the Healthy Living Center of Excellence at Elder Services of the Merrimack Valley in Lawrence, MA as well as Medical Director of Hebrew SeniorLife and Kendall Corporation’s Vitalize 360 Program. Prior to this, he served as the Chief Medical Officer and Physician-in-Chief of Hebrew SeniorLife in Roslindale, Massachusetts. He has served as a clinical instructor in medicine at Harvard Medical School in Boston for 21 years and a Senior Leader for the Practice Change Leaders Program.

Dr. Schreiber earned his Doctor of Medicine degree from Hahnemann Medical College following the completion of his Bachelor of Arts in psychology and biology from Dartmouth College.

In addition to his current work at Fallon, Dr. Schreiber serves as Senior Medical Advisor of Elder Services of the Merrimack Valley in Lawrence, Massachusetts and an advisor for the National Association of Area Agencies on Aging. He is also a member of the Public Policy Committee of the American Geriatrics Society and a member of the Innovations Platform Advisory Committee for the Society of Post-Acute Care and Long-Term Care Medicine. He is active in numerous professional societies including the American Geriatrics Society, Gerontological Society of America, the Society for Post-Acute and Long-Term Care Medicine,  Massachusetts Medical Society and American Society on Aging. Dr. Schreiber is a nationally recognized expert on integration of community-based services into medical care delivery systems and is regularly sought after for teaching and presenting to students and peers on regional, national and international levels.

In 2010, Dr. Schreiber received the Medical Director of the Year Award by the American Medical Directors Association and was honored as an American Geriatric Society Fellow in 2016.
